Appointments
Please arrive to the office fifteen minutes prior to your scheduled appointment time. Bring with you a list of current medications, allergies, contact information, referral doctor including his contact information. Have available drivers license, insurance cards / information, and pharmacy of your choice including current contact information. Wear no cosmetics, lotions, or sunscreens. Wear clothing that is easy to remove for skin examination.
